Pill May Reduce Hearing Loss From Noise

NEW YORK - Scientists have been accumulating evidence in lab animals for years that a pill might be able to reduce the damage loud noise does to your hearing. Research toward finding a pill that will help protect and even treat hearing loss from exposure to loud noise. While the effort is hardly new, experts say it has picked up steam in just the past few years. Nobody is saying such a pill could replace earplugs and other mechanical ear protection. But it's clear that the standard protections so far haven't prevented a wide-ranging problem.

Noise-induced hearing loss is one of the most common occupational diseases and the second most self-reported occupational illness or injury, the federal government says. About 10 million Americans have permanent hearing loss from loud noise, either a long-term exposure or in a sudden burst like an explosion. And an estimated 30 million people are thought to be exposed to hazardous levels of noise at work, such as in mining, construction, manufacturing and agriculture.

"Noise-induced hearing loss is such a common cause of hearing loss, and we haven't been very effective in ways to manage it," said Sharon Kujawa, director of audiology at the Massachusetts Eye and Ear Infirmary.

Scientists have pursued a variety of approaches toward an ear-fortifying pill. In 1994, for example, Israeli researchers reported that magnesium supplements helped military recruits avoid hearing loss over two months of noisy basic training. These days, much of the work focuses on antioxidants, the chemical class that most famously includes vitamins C and E.

That's because loud noise doesn't always damage the delicate inner ear immediately just by brute force. Rather, in most cases it provokes the inner ear into making harmful oxygen molecules called free radicals. Antioxidants neutralize free radicals, and the ear naturally has such defenses. But with enough noise, the ear's antioxidants are overwhelmed.

In that case, damage from the free radicals leads to death of the ear's sound receptors— hair cells, which convert the mechanical energy of incoming sound waves into nerve messages to the brain. As hair cells die off, hearing erodes permanently. So it makes sense to try to build up the ear's antioxidant defenses as a preventive step, and as a fast treatment after noise exposure. 
Animal experiments in prevention look promising, and "people are getting very excited about applying this to protection against noise exposure in humans," Kujawa said.

"I think it's very exciting," said Brenda Lonsbury-Martin, director of research at the American Speech-Language-Hearing Association. Antioxidants should work best if given prior to noise exposure, but animal work suggests it might reduce permanent hearing loss substantially if given within four to eight hours afterward, said researcher Col. Richard Kopke.

One pioneer in the field of using antioxidants for hearing has seen some effect in people. Dr. Michael Seidman of the Henry Ford Medical Center in Detroit got a patent in 1999 for a nutritional supplement he has recommended to several hundred patients for hearing loss caused by noise, medicines or aging. The patients have noticed an improvement in their hearing or a slowing in the progression of their hearing loss, Seidman said. For most with a moderate to severe hearing loss, that hasn't been enough to let them throw away their hearing aids, but it's often enough to let them turn theirs down or switch to a smaller and less powerful model, he said.  Specifically, he is recommending Body Language Vitamin Co.’s Anti-age/energy formula and the antioxidant formula.

Low carbohydrate diet shown to be of benefit

These authors randomly assigned 132 SEVERLY obese subjects (those with a body-mass index of 43 or above to a carbohydrate (CHO) restricted diet or a calorie and fat restricted diet.  Only 79 completed the six month study. Their results showed that sugjects on the low-CHO diet lost more weight than those on the low-fat diet (12.7 +/-18.9 lbs vs 4.2 +/-9.2 respectively…note the second number is the standard deviation-this was likely large because of the relatively small numbers of participants). The difference WAS highly statistically significantly different (p<0.001).  They also found an improvement in the insulin sensitivity and triglycerides levels, but they suggest caution in the interpretation because of the short duration of the study.  They also advised longer term outcome studies before they could formally recommend the CHO-restricted diet.

 

Drink your red wine and prevent hearing loss?

This study was designed to test the ability of resveratrol (a powerful antioxidant in red wine) to protect the ears from free radicals.  Oxidative stress (free radical damage) is known to cause MANY medical problems from Heart disease and Alzheimer’s disease to Cancer and aging.  In this particular study rats were given either 7 weeks of resveratrol (430 mcg/kg) per day whereas the control group received physiologic saline (ie salt water-but balanced to the body). The subjects were exposed to loud noise for 24 hours (the same level as would be seen at a rock concert). The long story short is that animals that were treated with the red-wine extract had much less hearing loss than the animals that drank a placebo (the salt water).  Moral of the story (perhaps a bit soon to tell, though Seidman doesn’t think so), is that a glass of red wine a day, might improve your hearing and possibly protect from the stresses of noise and free radicals.  What this study does not show, but is currently being studied in Seidman’s lab at Henry Ford Health System is the effect on aging and cognition, and the preliminary studies are looking very positive.

 

 

SOY and OTHER phytoestrogens containing compounds MAY be safe in the face of an estrogen sensitive cancer

Scientists and clinicians alike have typically recommended AGAINST the use of phytoestrogens (ie constituents of Soy-specifically dadzein and genistein, and black cohosh, wild yam etc) if you were to have an estrogen receptor positive breast cancer (as an example).  I have had several conversations with Dr. Mark Messina (a world authority on soy), who had mixed opinions re the safety of phytoestrogen use in the face of certain hormonally mediated cancers.  This paper suggests that the danger is probably not present.

 

Specifically, they note: “Estrogens found in plant foods (ie phytoestrogens), such as isoflavones found in soybeans and lignans found in whole grains, seeds and dried fruit, have shown to LOWER endogenous estrogen levels (Xu et al Cancer Epid. Biomarkers Prev 1998;7:1101-8; Lu et al Cancer Res 2000;60:4112-21; Hutchins et al Nutr Cancer 2001;39:58-65). Phytoestrogens also stimulate the production of sex hormone-binding globulin (SHBG) by the liver…”this leads to reduced amounts of estrogens available for binding with estrogen receptors. Phytoestrogens also bind competitively to estrogen receptors thereby blocking binding by other estrogens. Furthermore because of their weak estrogenic potential (<0.1% of that of estradiol), phytoestrogens DO NOT elicit a strong estrogenic response and thus have an antiestrogenic effect that INHIBITS the growth and proliferation of estrogen-dependent cancer cells (Molteni et al J Nutr 1995;125;751S-6S; Setchell et al Academic Press Limited; 1998 p 315-45).

In a nutshell, it may not be as dangerous as we once thought to use phytoestrogens in the face of an estrogen responsive tumor/cancer.

 

They concluded (Horn-Ross et al) that their findings support the hypothesis that phytoestrogens intake is associated with reduced risk of endometrial cancer, particularly in postmenopausal women, presumably through antiestrogenic effects.
